Apply to the Family Care Home Program
A short form — there is no fee to apply, and every program service is free.
Who should apply: families in — or relocating to — the San Francisco Bay Area with a child who has significant medical or special-education needs, seeking transitional housing, animal-assisted family visits, or help with family pet care. See the program page for full eligibility criteria.
How it works
- Download the application form below and fill it in — by hand or on your computer.
- Email it to board@heartsandpawsfoundation.org, with copies of your supporting documents. You can also post it to us if that is easier.
- We reply within five business days and tell you if anything else is needed.
- A review committee reviews complete applications within 30 days.
- You receive a written decision. If the home is occupied, you may join the waitlist and still receive other services as capacity allows.
What to have ready: one document from your child’s provider — a physician or hospital letter, treatment plan, IEP/IFSP, or regional-center letter (a copy, not the original). That is usually all we need to start. We will help you complete the form if you’d like, and we accept applications in your preferred language.

The form is three pages, and most families only need Sections A–D and the signature page. Send the completed copy — together with copies of your supporting documents, not the originals — to board@heartsandpawsfoundation.org.
If you cannot print or scan, email us and we will post you a form and a prepaid return envelope. We can also fill the form in with you over the phone and post you a copy to sign and return. Submitting an application does not guarantee selection.
Privacy: your information is confidential, is used only to determine eligibility and administer the program, and is reported publicly only in aggregate. Directors, officers, and contributors of the Foundation, and their family members, are not eligible, as federal tax law requires. The program does not discriminate on the basis of race, color, religion, national origin, ancestry, citizenship or immigration status, sex, gender identity or expression, sexual orientation, marital or familial status, age, medical condition, disability, veteran status, or source of income.
